What Is Class IV Laser Therapy?

Class IV laser therapy is a non-invasive treatment that uses a high-power therapeutic laser to deliver light energy into targeted muscles, tendons and joints. It is used to help manage pain and inflammation in certain musculoskeletal conditions.

"Class IV" describes the laser's power classification, not a treatment result. The light energy is absorbed by cells in the treated tissue, a process known as photobiomodulation.

Research suggests photobiomodulation may influence local circulation, inflammation and pain signaling. Responses vary, so treatment is planned around your diagnosis and reassessed during care.

Why Treatment Starts With an Evaluation

Two patients with pain in the same area can have very different causes. We identify the likely source first, because pain from soft tissue, a joint, a disc or an irritated nerve may each call for a different treatment.

  • Muscle, joint and inflammation-related pain may be evaluated for Class IV laser therapy.
  • Disc-related back or neck pain may call for spinal decompression.
  • Chronic tendon and plantar fascia problems may be evaluated for shockwave therapy.
  • Knee arthritis and stiffness may call for knee joint decompression.
  • Some patients need rehabilitation, a combined plan or another approach.

Joint Pain and Arthritis

Arthritis can cause joint pain, stiffness and swelling that worsen with activity or after periods of rest. Class IV laser therapy may be considered to help manage pain and inflammation in joints such as the hands, wrists, elbows and ankles.

Before recommending care, we look at the degree of joint degeneration and how your symptoms affect daily movement.

Back and Neck Pain

Laser therapy may be used for back and neck pain that comes from strained muscles, irritated soft tissue or localized inflammation. Pain that travels into an arm or leg can point to a disc or nerve problem instead.

When a herniated or bulging disc is suspected, DRX9000 spinal decompression may be a better fit, either alone or alongside laser therapy.

Knee, Shoulder and Hip Pain

Pain in these larger joints can come from arthritis, tendon irritation, soft-tissue injury or overuse. Laser therapy may be applied to the surrounding tissue to help manage pain and support movement.

Muscle and Tendon Conditions

What to Expect During Class IV Laser Therapy

  1. Evaluation. We review your symptoms, health history, previous care and any imaging, then examine the area to confirm laser therapy is appropriate.
  2. Preparation. The treatment area is uncovered and you are positioned comfortably, usually seated or on a treatment table.
  3. Protective eyewear. You and the clinician wear laser safety glasses for the entire session.
  4. Treatment application. The clinician moves the laser handpiece over the targeted area using a protocol selected for your condition. Many patients feel mild warmth.
  5. Reassessment. Your pain, movement and function are rechecked during care so the plan can be adjusted.

The number of visits depends on your diagnosis, the size of the treatment area and how you respond.

Treatment time depends on the size of the area and the protocol selected for your condition. Your provider will explain the expected visit length and schedule after your evaluation.
5How many laser therapy visits may be recommended?
The number and frequency of visits depend on your diagnosis, how long you have had symptoms and how you respond to care. We recommend a plan after your evaluation and reassess your progress along the way. No single visit count applies to every patient.
6Is Class IV laser therapy the same as cold laser therapy?
No. "Cold laser" usually refers to lower-power lasers, while Class IV lasers have a higher power output. Both use therapeutic light, but they differ in power, treatment technique and protocol.
7Who may not be a good candidate for laser therapy?
